Medical Policies
Physical Examinations
The State of Colorado recommends that each student have a dental and physical examination for each new, 1st, 4th, 7th and 9th grade student. The state requires vision and hearing examinations for kindergarten and grades 1, 2, 3, 5, 7 and 9.
Immunizations
Colorado law requires all students meet minimum
school immunization requirements. Any students who do not meet
the requirements cannot be admitted to school. At the time of registration, a certification of immunization must be presented to the school by
the parent or guardian. Students are not permitted to attend school
without the certificate. Students not current on immunizations will
have 14 days to obtain the next required vaccines or present a written
plan for completion. (A parent has the right to sign an exemption in
accordance with State law.)

*Any child starting or completing the series within
6 months of school entry may be certified with one less DPT and polio
vaccine.
Medications
Most prescriptions can be written to be given
outside of school hours. The only conditions under which designated
school personnel may administer any medications (prescription or
over-the-counter) are if the school administration receives each of the
following:
- A written statement from the physician detailing time
schedules, amount and method by which such medication (prescription or
over-the-counter) is to be taken and signed by the parent/guardian.
- A
written statement from the parent or guardian indicating the desire
that the school assist the student in the matters set forth in the
physician’s statement.
Vista Ridge Academy does not permit self-administration of drugs by students unless specified on the physician’s signed medication order. See numbers 1 & 2 above.
Medication must be brought in by the
parent/guardian in the original container bearing the original pharmacy
label for the specific student.
Medical Records
Medical records are kept in a separate, locked
file. HIPAA regulations on confidentiality of medical information
require that access to records be limited to office personnel, the home
room teacher, and the sports director. The release forms are carried
in a sealed envelope on trips and will be opened only in an emergency.
If there is specific information you want a teacher or sponsor to know,
please share it with them directly.
